Empowering and educating the unpaid caregiver is crucial to reducing costs and improving outcomes along every step of a Resident's Care Journey, and HealthHive provides the ideal infrastructure to facilitate it. We provide Residents a personalized digital care ecosystem that clearly incorporates their Providers’ suggested behaviors and activities while integrating the Resident’s feedback into the Providers’ existing systems. All this results in a longitudinal data-rich view of the Resident and those caring for them that will benefit Residents, Caregivers, Providers, and Payors.
The current structure of the US Healthcare system leaves HealthHive with an unlimited number of players and care environments that can and will benefit from our use, however we have chosen to initially focus on the care transition of the Skilled Nursing Facility to home.
As we look to the future we plan to take the data and best practices we learn through our work with the Skilled Nursing Facilities and use it to help any at risk party have a more powerful and meaningful relationship with their Resident’s support networks.
